Important Information
IMMUNIZATION FORMS (REQUIRED by the State of Tennessee)
If your student is new to the school, a Tennessee Immunization form (usually green) MUST be on file before school begins. If your student is a returning student, please be sure immunizations are current and properly recorded on the current form. If you have any questions, please call the office between 9AM and 3PM.

Medical Release Form
Student Data Form [custom form for your family was mailed to your home - to be returned by August 9].
Prescribed Medication Protocol
[to be used only when student needs to be given prescription medication during the school day].
Publicity & Swimming Permission Form

Speech and Hearing Evaluation Form
[required for all Level 1 students and students new-to-the-school, unless this evaluation has been done previously; if so, parent should provide a copy of results for teachers and office files].
